The Opportunity

For more than a decade, Springbrook has led efforts to provide coordinated, person-centered care for people with intellectual and developmental disabilities (I/DD). We have seen firsthand that when healthcare is consistent, compassionate, and designed to meet people where they are, lives are changed—individuals are healthier, families gain relief and trust, and communities grow stronger. Since opening the Springbrook Wellness Clinic in 2019, Springbrook has partnered with Bassett Healthcare to ensure patients receive coordinated primary care from trusted, consistent providers supported by integrated electronic health records. This work builds on a strong foundation, including the launch of a school-based health center at the Springbrook School that has reduced hospitalizations and emergency room visits among students. These enduring partnerships show that Springbrook is not testing a new idea—we are expanding a model that works. Expanding the Springbrook Wellness Clinic will allow more children, adults, and families to receive care from familiar providers in a setting designed for people with I/DD while also serving the broader community. Our experience shows that specialized primary care—across a lifetime—makes a measurable difference.

Expanding Access

We have received final approval from the Department of Health to expand the existing clinic into an Article 28 facility (a NYS Department of Health-certified Diagnostic and Treatment center). The campaign will expand and renovate the existing clinic, centrally located on our Oneonta campus next to the gym and conveniently accessible from I-88 Exit 16.

The expanded clinic will provide:

  • Comprehensive, sensory-responsive primary care for adults and children

  • Integrated behavioral health services

  • Extended appointment times

  • Coordinated care and transportation support

  • Trauma-informed, disability-competent staff

This expansion increases access while maintaining the familiarity, expertise, and compassionate environment our community trusts.

Expanding Hope

Expanding Access, Expanding Hope is a $2.5 million campaign to renovate and expand the Springbrook Wellness Clinic.

Campaign investments will support:

  • Article 28–compliant renovations

  • New exam and clinical spaces

  • Integrated behavioral health areas

  • Sensory-responsive design

  • Accessibility enhancements

  • Technology and infrastructure upgrades

Every investment directly strengthens access, dignity, and health outcomes for people with I/DD and for the surrounding community.

Impact

With expansion, Springbrook will significantly increase the number of patients served and visits delivered annually—growing from 2,000 visits in year one to more than 7,200 visits by year five.
